A S Tax Service LLC must stay compliant with the IRS; Tax Identification Numbers are mandatory items on your checklist.
All taxpayers will need the following to do their taxes.

Unexpired ID (front & back).

Your social security card or tax ID number.

Your spouse’s unexpired ID and social security card.

Please provide your Identity Protection PIN if the IRS has issued one to you, your spouse, or your dependent.

The bank name, routing, and account numbers are used to receive your refund by direct deposit.
Dependent(s) Information

Social security card(s) for everyone on tax return.

Birth certificate (optional)
Sources of Income
Please provide ALL tax forms that show earned and unearned income.

1098’s,

1099’s,

Social Security, SSI Benefits, State Assistance

Self-Employed (PLEASE schedule a free call with me prior to any self-employed returns)
Childcare Expenses
Fees are paid to a licensed daycare center or family daycare for the care of an infant preschooler or child up to age 12.

MUST have signed letterhead from a provider with the child’s name, provider name, business EIN or social number, dates, and amount paid out of pocket.
Health Insurance

Form 1095-A IF you enrolled in an insurance plan through the Marketplace (Exchange).
Educational Expenses

Forms 1098-T from educational institutions. Receipts for all out-of-pocket school expenses.
MICHIGAN RESIDENTS ONLY

If you paid rent during the tax year, provide your landlord's name and address, dates of rent, and payments paid out of your pocket.
Utility bill dated JANUARY 2024, pages 1 & 2 for Michigan Home Heating Credit
